2012  Portfolio Requirements
 
All students will be expected to complete a portfolio and to participate in an Exit interview before graduating from Cambridge High School. The portfolio should reflect some of the experiences you have had while attending Cambridge High School. Use a three-ring binder to collect these materials. 
 
Portfolios are due on April 9, 2012.
 
Requirements for your Senior Portfolio:
(Please place these items in your portfolio in the order that they are listed below)
 
¨      Cover Page: The cover page should have your name and a title “Senior Portfolio” listed. Many students often include a picture/senior picture on this introduction.
 
¨      Cover letter: The cover letter states the purpose of your senior portfolio
 
¨      Résumé: must be a current résumé
 
¨      Personal information sheet:
 
¨      Individual Learning Plan: 4 year plan (listing the classes you have taken at CHS)
 
¨      Co-curricular participation summary
 
¨      Non-school participation summary
 
¨      Senior Project: Democracy and Me paper (from Government and Economics Class)
 
¨      Personal goals sheets: one representing each year at high school
 
¨      Goal Reflection Summaries: 1 short reflection paragraph for each grade level goal
 
¨      2 recommendations from teachers
 
¨      Additional Recommendation letter: (from employer)
*If you have not had a job, or if you are currently unemployed and do not have access to your former employer it is recommended that you ask for a letter of recommendation from someone other then a teacher. You can ask a coach, a member of your church, someone you volunteered for, etc. You will receive no credit if you do not submit an extra recommendation.
 
¨      Academic areas:
Select a minimum of one activity or homework sample from each core academic area.
1.      Math
2.      Science
3.      Language Arts
4.      Social Studies
Reflections should be attached to each activity or homework sample. The reflections should describe the following questions:
-          Course Title and year the work was completed
-          What was the purpose of the assignment?
-          Why was the assignment included in the portfolio?
-          What was learned by doing this work?


¨      Elective area classes:

Select and reflect upon three samples of work from the following areas:
1.      Art
2.      Business Education
3.      Agriculture
4.      Family and Consumer Sciences
5.      Foreign Language
6.      Music
7.      Physical Education
8.      Technology Education
 
Reflections should be attached to each activity or homework sample. The reflections should describe the following questions:
 
-          Course Title and year the work was completed
-          What was the purpose of the assignment?
-          Why was the assignment included in the portfolio?
-          What was learned by doing this work?
 
¨      Other class work, Club or Extra curricular involvement, Hobbies, Other Interests:
 
 A minimum of 3 examples is required of additional information:
Examples:
§                    National Honor Society or other club affiliations
§                    samples of artwork or photography,
§                    photographs of class projects
§                    certificates
§                    awards
§                    newspaper clippings about yourself
§                    compositions (essays, poems, reports)
§                    presentations
§                    music solos
§                     any other related item that provides information about you
 
¨        The following items are optional but have been common in Senior Portfolios:
 
§                    Report cards
§                    Transcripts
§                    Standardized Test Scores (make sure you black out Social Security Numbers)
§                    Activities, Awards and accomplishments
§                    Programs from athletic events or other school events you have participated in
§                    Projects
§                    Any other things you would “show off” to an employer/committee
